To qualify, proof of a design-based profession must be submitted. As proof, please submit a copy of valid membership to:
• ASID American Society of Interior Designers or international affiliate
• IIDA International Interior Design Association
• AIA American Institute of Architects
• IDS Interior Design Society
• CID Certified Interior Decorators International
• National Council for Interior Design Qualification Certificate Number

· Copy of design affiliation membership card/certificate

· State Business License

· Business Card

· Yellow page advertisement

· Two recent invoices
